You must have a primary keyword for each page and not more than two other closely related supporting keywords.
Or read this link about the place of keywords.

It is better not to repeat the keywords in almost all the pages. other wise the most important page will be ignored by the search engine.

Use the keyword in the title and in the content. Remember, you should not use the keyword more than thrice within the content. Search engine spiders look out for titles first. You gotta post competitive keywords there.

Where to place your keywords? Definitely in Meta Tags.
Where is the best? Title Tag
But remember to avoid keyword stuffing. It would definitely hurt your site. :D

When you put your keyword in your content, try to come up with a good writing where you can embed your targeted keywords. That way your message will still look natural and as the others say, out them in your meta tags placing your most important keyword first.. :D
